This one-day conference brings together educators, families, healthcare providers, community leaders, service agencies, and individuals with disabilities to explore how collaborative networks can transform the transition to adulthood. Through focused sessions on school, healthcare, community, and employment, participants will learn practical strategies and hear from diverse voices in dynamic panel discussions —building the connections needed for lifelong success. Additionally, conference attendees will have access to our Community Partner Fair, comprised of local and state-level businesses, service providers, and organizations geared towards supporting one’s transition to adulthood.
