By completing this rental request, I accept full responsibility for the items checked out. In the event of theft or damage, I understand I am financially liable for repair or replacement of these items. Upon evaluation and determination of financial responsibility, I will remit payment (to Baylor University) within ten business days of evaluation so repair or replacement can be expedited appropriately.
